CPT 99483 — the comprehensive cognitive assessment
A one-time (per 180 days) comprehensive assessment and written care plan for a patient with cognitive impairment, with nine assessment elements plus the written care plan. It is an assessment-and-plan service, not ongoing management.
99490 — Chronic Care Management (CCM)
99490 is ongoing, monthly care management for patients with two or more chronic conditions — roughly 20 minutes of clinical staff time per month under a care plan. It is recurring management, not the one-time comprehensive cognitive assessment 99483 represents, and the two can be complementary across a patient’s care.
99484 — Behavioral Health Integration (BHI)
99484 covers monthly care management for behavioral health conditions using general BHI. It is a different clinical domain and a monthly service, distinct from the one-time cognitive assessment of 99483.
G0505 — the predecessor
G0505 was the earlier HCPCS code for cognitive assessment and care planning; it was replaced by CPT 99483 in 2018. References to G0505 in older material correspond to what is now billed as 99483.
