CPT 99483 documentation requirements
Billing CPT 99483 correctly depends on documentation. The note must show that each required element was addressed; where an element does not apply, that should be recorded rather than left blank.
Document every required element
The encounter note must evidence all nine assessment elements and the written care plan. Missing or undocumented elements are the most common reason a 99483 claim does not withstand review.
Name the standardized instruments and their results
- A validated instrument used for dementia staging (for example FAST or CDR).
- A standardized depression screen (for example PHQ-9 or the Geriatric Depression Scale).
- Record the instrument used and its result — not just a narrative impression.
Capture time, complexity and the historian
Document medical decision making of moderate-to-high complexity, the involvement of an independent historian, the setting, and the time spent where time is used to support the service.
The written care plan is part of the record
The care plan is a required, documented output — it must address neuropsychiatric and neurocognitive symptoms and functional limitations, and be shared with the patient and/or caregiver. Keep evidence that it was created and provided.
If an element does not apply
When a required element is not applicable to a patient (for example advance care planning the patient declines), document that it was considered and why it does not apply, rather than omitting it silently.
